Passports, private medical certificates and similar forms can often be completed and signed by the doctor without the need to be seen. These are private services and the doctor may decline to complete any form.
Where an insurance company or solicitor requests a report the practice provides a copy of the complete patient record, providing patient consent is received, to the insurance company or solicitor.
There will be a charge for all private services.
