The practice offers NHS health checks to eligible patients within the specified age range, 40-74 years old.
One check may be given in any 5 year period.
Patients aged 75 or over may request an over 75 health check with a health care assistant or practice nurse. If you would like one, please ring the surgery to book an appointment.
